​​Our clients in South London are urgently seeking a Band 7 Liaison Practitioner to join their Older Adults Consultation and Liaison Service
Hours: Monday- Friday 9am-5pm
Job Description
  • We provide psychiatric assessment and review for patients aged 65 and over (not active to working age mental health care services) who have been admitted to Hospital. We work jointly with our general liaison psychiatry colleagues providing mental health input to older patients presenting to Emergency Department.
  • The service facilitate mental health follow-ups, where indicated by referral to the appropriate community mental health, Home treatment team and Care home intervention team.
  • The older adult mental health liaison team at the hospital provides specialist mental health assessment and support to the inpatient wards and emergency department for all patients from age 65 upwards.
  • Successful applicants will join a highly experienced and dedicated team who pride themselves on collaborative working with service users, carers and other professionals who provide a specialist assessment and management of care of older people.
  • The successful candidate will also have an excellent opportunity to build upon their established clinical and interpersonal skills by working with different health care professionals, learning each other roles and function of team.
  • The successful applicant will hold an appropriate professional registration, will have a strong proven clinical knowledge and demonstrate on-going professional development. Previous experience of working with older adults is essential. The role will include use of computer systems so proficient use of IT is necessary. Working with complex patients presenting with comorbidity issues and you will have opportunity to build on your existing knowledge.
  • You will hold a sound working knowledge of common mental health disorders, the Mental Health Act and Mental Capacity Act & DOLs. A sound knowledge of the interface between physical and mental health is essential as well as experience of working with older adults, in any setting.
