​​​Our clients in Barnet, Enfield and Haringey are urgently seeking to recruit a Band 7 Case Manager to join their Learning Disability and Autism Service.
Hours: Monday to Friday 9am to 5pm.
Job Summary
The post holder will be part of the Commissioning Hub, which is responsible for the quality, performance and finance of all NLFC providers and all patients who originate from North London regardless of which provider the placement is with.
The post holder will work as part of a clinical team with a defined caseload of Learning Disability & Autism (LDA) patients. The majority of LDA patients will be placed within North London providers but some will be placed outside of London in other NHS or Independent Sector Placements. The post holder will undertake 8 weekly commissioner oversight visits of placements, reviewing service experience and pathways for a defined caseload. The post holder will ensure the recommendations of the CTR are being acted upon and patients are progressing through their pathway. The post holder will play a key quality assurance role covering areas such as serious incidents, safeguarding and complaints and supporting with quality improvement initiatives.
