Our clients in Oxford are urgently seeking to recruit a Band 6 Social Worker to join their Community Mental Health Team.
Shift Pattern: Monday to Friday 9am to 5pm.
To work in an integrated health and social care team that supports individuals with mental health problems.
Assess the needs of people with mental health problems under the Care Act 2014 from a Strengths Based perspective and create care plans to meet their needs.
Assess capacity under the Mental Capacity Act 2005 and following due process (e.g. Best Interests Meetings).
To have knowledge in relation to the powers of the Mental Health Act, the implications for S117 aftercare, and the ways in which detention in hospital can impact on people with mental health problems.
Establish working relationships with people with mental health problems, carers, and fellow professionals.
Identify Safeguarding concerns (including where a S42 Enquiry is required) and able to follow the relevant process to support people at risk of abuse or neglect (including supporting colleagues to do so).
To assess, arrange and review care packages for those who are eligible for support under the Care Act 2014 or under 117 aftercare arrangements (Mental Health Act 1983)
The following skills have been specified: -
Experienced Social Workers preferably with mental health experience
Use of electronic notes systems - LAS & Care Notes desirable but not essential
Car is required.
