​​​Our clients in South London are urgently seeking to recruit a Locum Band 6 Community Mental Health Practitioner to join their Older Adults Home Treatment Service.
Hours: Monday to Sunday - Earlys 9am to 5pm, Lates 1pm to 9pm, Weekends 10am to 6pm.
Job Summary
  • The post holder will be a member of the Home Treatment Team. They will also have responsibility of working some night shifts within the integrated out of hours services which may entail undertaking assessments or offering support across geographical borders within the SLP.
  • The main target group are people with severe mental health problems experiencing mental health crisis.
  • The Home Treatment Team is available to provide care and intervention 24 hours a day, 365 days of the year for people who are experiencing a mental health crisis of a nature or degree that would require admission in the absence of the CHTT/CMHT.
Clinical Requirements
  • Engaging clients in crisis
  • Participating in rapid assessments at home, at community trust sites and in Accident and Emergency Departments.
  • Co-ordinating effective gate keeping assessments when needed.
  • Providing a range of interventions for service users and carers.
  • Planning for robust and sustainable discharge and aftercare.
  • To work collaboratively with service users and carers to co-produce crisis plans
  • Working as part of the team to achieve the best possible care for clients.
  • To offer advice and support to other professionals on issues of referrals to HTT or signposting if not appropriate for HTT
  • To maintain and improve own professional, clinical, managerial, knowledge and skills by promoting self growth and development through the in-service training program, debriefing sessions following SUIs,
  • Reflective Practice, and other educational program/materials as agreed by the manager.
  • To work flexibly in ensuring maximum efforts are made to engage service users/carers.
  • To undertake home visits (single or joint) single visits only when there are no health, risk or safety issues evident.
  • The post holder will comply with Trust-wide Lone Working Policies and local agreed Operational Procedures with regards to visits in the community.
  • Assessing capacity and consent related to care and treatment options.
  • The Home Treatment teams are not aimed at providing interventions for people whose primary need is drug or alcohol dependency, learning disability, brain damage or dementia, or children and young people under 18 years of age.
