Our clients in Central North West London are urgently seeking a Band 6 Discharge Co-ordinator to join their Older Adult Inpatient Service.
Hours: Monday to Friday 9am to 5pm .
Job Description
The post holder will primarily be based on a psychiatric inpatient ward for older adults, under the supervision of the local OP CMHT (who deliver health and social care on behalf of local councils). The post holder will bring social work expertise and knowledge to the service to help facilitate and co-ordinate timely discharges, taking action to expedite the process, avoid delays and thereby improve the patients’ experience, including supporting discharges for patients who have been admitted from outer boroughs. A critical success factor in this role is reducing the length of stay. The Care Manager will work closely with colleagues within the ward and CMHT and be an active part of a ward multi-disciplinary team, dealing with challenging hospital discharges. The post holder will need to demonstrate strong initiative, problem solving skills and confidence in order to navigate the various processes, assessments and funding panels, supporting ward staff with assessments for discharge and a good knowledge of S117 processes, the MHA, CPA, MCA and BI processes, and developing relationships with local providers. The post holder will ideally have experience of working with older adults and working within a mental health setting.
