Our clients in Oxford are urgently seeking to recruit a Locum Band 6 CPN to join their You Matter Service.
Shift Pattern: 37.5 hours per week, the post holder will be expected to work flexible hours to cover the shift patterns (8am to 4pm and 12pm to 8pm) as required by the service.
The post holder will carry out the mental health assessments of clients / staff members who contact the staff mental health and wellbeing hub, known as You Matter. To manage, formulate and implement treatment plans, delivering care under the clinical model. To evaluate effectiveness whilst working within the team process and Risk Assessment. To assess, plan, set goals with the clients / staff members, carer and families; implement and evaluate care using psycho-social interventions and skills to meet individual needs. To be responsible for monitoring and evaluating treatment plans, transferring clients / staff members to the treatment function, other teams within the Trust, or discharging them when appropriate. To liaise and establish good working relationships with referring agents, colleagues in Primary Care services, Social Services, Community Mental Health Teams, Occupational Health, Professional Bodies, Voluntary Sector and other agencies as appropriate. To be an active member of the multi-disciplinary team and to attend case management supervision and other work led meetings as required. To maintain clients / staff members confidentiality in delivery of treatment packages. To respect the needs of clients / staff members, their families and other significant persons in care planning and management, including assisting them in meeting their needs.
