Our clients in West London are urgently seeking to recruit a Locum Band 6 CPN/Social Worker or Occupational Therapist to join their Primary Care Service.
Hours: Monday to Friday 9-5.

Job Summary
​This role is based on delivering of the work detailed in the Shaping Healthier Future – Shifting Settings Business Case. This will involve a number of elements which will support the management of people with mental illness in primary care, who would have been previously managed in secondary care mental health services. This role will focus primarily on facilitating discharge from secondary care, but also accept referrals from primary care to prevent referral into secondary care. This will be achieved by providing support in primary care. The post holder will be part of a small team which will cover the whole borough and as such each post holder will be part of the wider primary care locality team.
The objectives of the services are:
  • To facilitate the number of people with long term mental health conditions discharged back to their GP from the Trust.
  • To provide an enhanced level of support to PC Clinicians through advice and training to improve GP skills and capacity in responding to the needs of people with mental health conditions discharged back to them.
  • To work within an agreed process for safe discharge from the Trust to GPs as well as rapid consultant advice and rapid re-referral, when appropriate.
  • That patients are discharged to their GP under this process have good clinical outcomes.
  • That patients discharged to their GPs under this process have access to appropriate advice, advocacy, vocational support to access social networks.
  • That the Public Sector Equalities Duty is met.
