Our clients in South London are urgently seeking 2 Band 6 Community Mental Health Nurse (CPN) to join their Primary Care Team
Hours: 1 Band 6 CPN required Monday to Friday 9am to 5pm and 2nd Band 6 CPN required for 3 days per week between the hours of Monday-Friday 9am-5pm.
Job Description
The Promoting Primary Care Team provides care and support for people, aged 18-65, who have severe mental illness. They care and support people who live in the local borough.
People eligible for the service must be on the Care Programme Approach (CPA). The CPA is a way of describing what you need in order to get the best care. It is for patients who need to see several people or organisations for their mental health care and treatment.
The service provides people with a care coordinator to plan their care and make sure they get the best available help. They are usually a community psychiatric nurse, social worker or occupational therapist. They also run an outpatient clinic for people who are able to live more independently.
Their service brings together teams of health and social care professionals, including nurses, social workers and psychiatrists.
