Our clients in Kent are urgently seeking to recruit a
Locum Band 6 Community Psychiatric Nurse to join their Primary Care Mental Health Team.
Hours: Monday-Friday 9am to 5pm
Job Summary
This role will involve several elements which will support the management of people with mental health conditions in primary care who would have been previously managed by secondary care mental health services. In the first instance the focus will be on facilitating discharge from secondary care and providing support in primary care. The post holder will be part of a small team which will cover a whole borough and as such each post holder will be part of the wider locality team.
The objectives of the services are:
To facilitate the numbers o people with long term mental health conditions discharged back to their GP from the Trust
Provide an enhanced level of support to PC clinicians through advice and training to improve skills and capacity in responding to the needs of people with mental health conditions discharged back to them
To work within an agreed process for safe discharge from the Trust to GPs as well as rapid referral when appropriate
That patients discharged to their GP under this process have good clinical outcomes
That patients discharged to their GPs under this process have access to appropriate advice, advocacy, vocational support to access social workers
That the Public Sector Equalities Duty is met
