Our clients in South East London are urgently seeking a Band 6 Community Mental Health Nurse (CPN) to join their Mental Health Liaison Service.
Hours: The team operates on a 24 hour, 7 day a week rota and it is essential that the post holder is able to work unsocial hours, including nightshifts and weekends on a pro rata basis.
The Mental Health Liaison Team is looking to recruit an enthusiastic, motivated and highly skilled practitioner.
A sound knowledge of the interface between physical and mental health is essential.
Experience of working with older adults, in any setting, as well as those who use illicit substances and/ or misuse alcohol is highly desirable.
All applicants will also hold the relevant mentorship qualification or be willing to undertake this training.
The post holder will be expected to identify and assess adult patients of all ages presenting to the Emergency Department in acute mental health crisis and provide assessment for inpatients on the acute wards at the hospital. The post holder will work alongside acute trust colleagues and other partner agencies to develop plans of care alongside the patient and their carers, relatives or friends.
The post holder will be expected to be an advocate for mental health within the hospital and support all staff in providing care for people presenting with complex and multifaceted difficulties. The team work alongside Mental Health for Older Adults, Perinatal Mental Health and Neuropsychiatry team.
The post holder will be expected to provide assessment and care to patients presenting from these specialist areas, out-of-hours.
The post holder will be expected to undertake rigorous data collection in order to evidence the clinical effectiveness of the service.
