Job Description
The Home Treatment Team provides 24hr, 7 day a week, crisis assessment and intensive home treatment to patients with the aim of preventing in-patient admission where possible or if admission is required facilitating earlier discharge.
The post holder will work as part of the borough’s Home Treatment services on a rota basis, 7 days a week, 7.5 hours a day. The role will include working across the Trust’s Acute Referral Centre (ARC) on a rotation basis, as the ARC operate 24 hours a day 7 days a week, although the majority of time is spent located in the borough Home Treatment Service.
When working in ARC Practitioners undertake triage assessments and on occasions face to face assessments. They form part of a team that manages beds and are expected to follow through the referral to a safe conclusion which means liaising with wards, community services and Home Treatment teams. They are active members of the bed management coordination function for the Trust and as such get involved with the entire booking process for in-patient admissions.
When working in the borough HTT team, practitioners undertake assessments and treatment as required. The person will lead in the assessment, co-ordination, planning, implementation, evaluation and delivery of high quality home treatment for people in acute psychiatric crisis. The role will involve responsibility for an allocated group of mental health service users in crisis, with the aim of promoting their recovery and maintaining them in a community setting rather than in hospital.
The post holder will play a key role in the current development of the team and help implement the service’s aims and objectives, as well as helping to ensure good links with referring in patient and community teams, with a view to reducing the number and length of inpatient admissions. Effective gatekeeping assessment and the identification and delivery of the best treatment packages for people presenting to services in crisis will also be key.
Previous Home Treatment Team or Assessment experience is essential to be considered for this role.
