​​Our clients in South East London are urgently seeking 2 Band 6 Community Mental Health Nurses (CPN) and 1 Band 7 Community Mental Health Nurse (CPN) to join their Community Treatment Team
Hours: Monday- Fri 9-5
Band 6 CPN Job Description:
  • To work as a highly experienced autonomous practitioner carrying out specialist needs assessments, care planning, implementation and evaluation to a significant number of adults referred to the Treatment Team. The Senior Practitioner will assess and formulate a care plan and offer brief interventions (up to 12 weeks) with mental health service users, signpost to other services and discharge to primary care where appropriate/possible and supervise community practitioners. Communications and Working Relationships: Multidisciplinary Team, other borough teams, external agencies and primary care - daily.
  • To assess needs, plan, implement and monitor care in collaboration with the service user, carers, and other members of the multidisciplinary team.
  • To implement care that meets the mental, physical, spiritual, psychological and social needs of the service user, and that is sensitive to age, culture, race, gender, ethnicity, social class, sexuality and disability.
  • To administer medication with knowledge of contraindications and side effects, and relating accurate information to service users in a format they are able to understand.
  • To formulate needs assessments for service users requiring commissioned services and to monitor care packages as directed.
  • To ensure that risk assessments are completed in full, documented and actioned.
  • To provide continuing assessment and follow up of the individualised care plan, liaising with appropriate stakeholders.
  • To demonstrate an understanding of the distress caused by, and social and personal consequences of mental illness for service users and their carers.
  • To formulate plans of care with service users which actively promote activities of health promotion and recovery.
  • To work within policy, professional and legal frameworks at all times. Understand and upholds knowledge of relevant Code of Professional Conduct, risk assessment and child protection protocols, Mental Health Act and CPA, and Trust clinical policies. Facilitate the service user’s choice, within legal, ethical and professional framework.
  • To work with in an appropriate psychological framework with service users and carers.
Band 7 CPN Job Description:
  • To have responsibility of holding a complex caseload within an integrated community mental health team, in accordance with Trust policies and procedures.

  • To support the Team Leader and Service Manager to lead a team of professional and support staff in the delivery of high quality evidence-based practice, ensuring that regular supervision and appraisal take place.

  • To manage administrative and operational resources in a cost-effective manner, evaluating the standard of service delivered.

  • To support with co-ordinating multidisciplinary working, maintaining excellent communication and working relationships with all team members, and other services within the

  • Trust, to ensure effective interface and continuity of care between disciplines and services.

  • The post holder will carry out clinical and supervisory procedures to a standard that ensures safe and effective care, and complies with the relevant Code of Professional Conduct.

  • There is an expectation that the post holder will place the needs of service users at the centre of care delivery.

  • To support the Team Leader and Service Manager with the Community Transformation Programme within the borough and be involved with supporting the team, patients and their carers with the changes ahead.
