Our clients in Essex are urgently seeking to recruit a Locum Band 6 and a Locum Band 7 CPN to join their First Response Team.
Hours: Monday to Friday 9am to 5pm.
To be responsible as the Care Coordinator for the assessment and coordination of care needs, planning and implementation of evidence based care to a defined group of service users and their carers within a defined catchment area/locality. To provide professional advice and support to other agencies and members of the multidisciplinary team, and act up for the Team Lead in their absence as appropriate. To act as a role model and resource for junior/less experienced staff.
The post holder will be responsible and provide effective leadership for a clinical team providing a high quality service within the resources available within a defined area, and specifically to deliver on the following key areas:
To co-ordinate and lead team functioning with particular focus on:
• Management of referrals including delivery of care
• Performance monitoring and management
• Line management and supervision of staff
The post holder will be responsible for ensuring delivery of high quality, recovery focused, appropriately targeted services which operate in accordance with agreed plans and objectives. The post holder will have a good awareness of current mental health and social care issues which affect or influence the community and care of adults with mental health needs.
